Courtrooms figured out a century ago that a lawyer can't argue and take notes at the same time, so they hired a stenographer. Medicine, meanwhile, gave every doctor a keyboard and told them to make eye contact anyway. The VA just moved to fix that, and the stenographer is a robot.
On September 22, Abridge announced it had been selected to provide ambient AI under a Department of Veterans Affairs enterprise contract. An ambient AI scribe is software that sits in the exam room, listens to the conversation, and drafts the clinical note so the doctor doesn't have to, which is a sentence that should make you nervous and grateful in equal measure. It hears you admit you drink more than you told them last time and files it under Social History without judgment, which is more than we can say for your brother-in-law.
About that $775 million. It's the ceiling on a five-year, multiple-award contract shared across every eligible vendor, an Indefinite Delivery, Indefinite Quantity deal, which is government for "we'll buy as much as we need whenever we need it." So it's less "the VA wrote Abridge a giant check" and more "the VA opened a very large tab and several vendors can order from it," a distinction that's easy to lose in a headline. Individual VA medical centers and regional networks place the actual orders.
Abridge is already live at more than 75 VA medical centers after a pilot, across primary care, a dozen specialties, and the VA's virtual care hubs, and the company says it works in more than 28 languages. The scale is the point: according to Abridge's announcement, the VA serves more than 9 million veterans across 1,380 facilities, and it's in the middle of one of the largest electronic health record transitions in history, meaning clinicians are juggling an old system and a new one. Abridge says its scribe works in both, which puts it ahead of every human who has ever been asked to use two systems at once.
The VA didn't hire a robot to replace doctors. It hired a stenographer, so the robot gets the transcript and the doctor gets the eye contact. Somewhere, a physician is about to make full eye contact for fifteen straight minutes, and it's going to be weird for everyone.
